New Patients


If time permits, we still send new patients a new patient packet and other materials to collect the information necessary for the office visit. However, our patient forms can be downloaded and printed by clicking below. Arriving to your appointment with the forms already filled out will save time. If you are unable to complete a new patient packet before your appointment, we recommend you arrive at least 30 minutes before your scheduled appointment to allow adequate time for completion.

It is important to bring all pertinent films and records with you, or make sure they have been sent ahead of time. Most referred patients have had MRI or CT scans and/or other tests done that have prompted the referral to a neurosurgeon. Our evaluation and management is heavily dependent on the findings on those tests and often we cannot complete the consultation without them. Since MRI and CT scans are done in many different formats by various radiologists, it is also very important to get the reports.  You can either bring them with you or be sure they are sent to us (usually by fax) ahead of time.  Remember, failure to get all of the appropriate information may result in rescheduling your clinic appointment.

When you call for an appointment, we will try to obtain necessary information about your insurance and type of coverage. With worker's compensation patients and especially with managed care patients (PPO or HMO), we often must get pre-authorization numbers before your visit.

For your convenience, the office is open from 8:30 a.m. to 5:00 p.m. Monday through Thursday and 8:30 a.m. to 4:00 p.m. on Friday.  We ask that you give us at least 24 hours notice if you are not able to keep your appointment.
