and phone number

Referrals from your primary care physician

Insurance cards

Drivers License
-
Films or CD of your most recent MRI,CT or X-rays including
their radiologist report

Any previous medical records for example EMG reports or bone scans

If you have any Advance Directive-Living Will, please bring for your
chart
OFFICE POLICY

If unable to keep appointment please give us a 24 hour
notice, failure to notify will result in a $50.00 charge.

Please arrive 15-20 minutes prior to your appointment time

Co pay is due at time of service.

We DO NOT CALL PRESCRIPTIONS any
time after office hours. This includes evening and weekends. Prescriptions
are refilled Monday – Friday from 9am to 4pm !!

Must advise before appointment
date if this is related to workman’s
compensation or a motor vehicle accident with pending litigation. You
may be required to make full payment for your visit or your appointment
rescheduled until information or authorization has been obtained.
